Items to be completed via ONLINE registration BEFORE your first practice:

1. Physical dated after 4-1-22
2. Emergency Contact/Insurance Form
3. Handbook Acknowledgement Form electronically signed by parent and athlete 
4. Concussion Acknowledgement Form
5. $50 fee 1st sport, $40 fee 2nd sport, $30 fee 3rd sport (Payments can be made in the High School or Middle School main office or online.)
